* Why do you want to foster dogs?
* Briefly describe your lifestyle. Are you an active family? Are you couch potatoes? How would a dog fit into your lifestyle?
* Do you travel often? If so, what do you plan to do with the dog while you are away?
* On average, how long will the dog be left alone each day?
* Where will the dog be kept when you are not home? For example, outside in yard, outside in pen, in crate, inside free, or inside in an enclosed area.

If you do not have a fenced-in yard, how do you plan to exercise the dog?
If you do have a fenced-in yard, how else will you exercise your dog?

Please inform us about your current pets: include Type, Breed, Name, Male/Female, whether Spayed/Neutered, Age, Years with your family, and Where does he spend most of his time, temperament with dogs, and temperament with people. It is important that you include answers to ALL of these questions.
Please list pets that you have had in the past, (Pets you no longer have in your household): Include Type, Breed, Name, Male/Female, Whether Spayed/Neutered, Age, From what date to what date did you have this pet, Why is this pet no longer with you. It is important that you include answers to ALL of these questions.
Please give us information about your veterinarian(s): Include, Name of animal hospital/clinic, Town/State, Phone Number, Is your pet up to date with vaccines, Whose name is the pet(s) listed under, How often do you take your pets to the vetrinarian? It is important that you include answers to ALL of these questions.

* Are there any breeds of dogs that you do not feel comfortable fostering? If yes, please explain why:
* Do you understand that every new foster dog will need time to adjust to his new environment? Each dog will be a different experience and will require much of your time and patience until he adjusts. Are you ready to make this commitment? Please explain how you feel about this:
* Many dogs are rescued from shelters and may have unknown histories. Temperament tests do not guarantee the behavior of the dog once he is in your home. How do you feel about this?
* Do you plan to train your foster dogs? How experienced are you with training? Are you familiar with pack order and the alpha dog?
* How often and how will you exercise your foster dog?
* Will you allow your foster dog on the furniture?
* Will you walk your foster dog off-leash? If yes, please explain:
* Are there any habits that you will not tolerate from your foster dog? (For example, barking, chewing, etc.)
* Do you feel comfortable fostering a dog that may not be housebroken?
* How do you plan on disciplining each dog? (For example, a smack on the nose, positive reinforcement, a firm voice, or a combination.)
* Would you describe yourself as a pushover or a firm disciplinarian?
* Foster parents will be responsible for the cost of food, toys and accessories. Can you afford this?
* Some foster dogs may not get adopted as quickly as others (it may vary as much as a few weeks to several months). Are you prepared to foster a dog for the entire time, until he is adopted?
* Some dogs may have to go into foster care before veterinary needs are met. Are you willing to transport the dog to and from a veterinary facility for initial veterinary care, spay/neuter surgery and/or vaccine boosters?
